We are located at the corner of Naomi and Alameda, which is almost right across the street from Providence St. Joseph Hospital.

Parking
Parking is available in the parking structure adjacent to the office building. You may enter our parking from Naomi. Parking fees do apply; we do not validate. Alternatively, you can find street parking.

If you are a new patient, an existing patient with a new injury / pain, or if your insurance has changed, please complete our patient forms.

Things to Bring & Policies To Read

• Your valid health insurance card
• Payment (Please read our Insurance & Fees Policy)
• Your driver’s license (or valid picture ID)
• A complete and accurate list of your current medications
• Referring doctor contact information so our doctors may send office notes and a letter after your visit
• Any tests (x-rays, CT scans, MRIs) relevant to your problem
• Completed New Patient Forms

Your Time
